myNurse BackgrounderThe myNurse service includes: Personal health and wellness plan – All clients who purchase myNurse receive a personalized health and wellness plan during the first visit. In subsequent visits the nurse monitors the client's health and wellness to determine if they're achieving their plan objectives. On-going health and wellness monitoring – Regular home visits provide a thorough evaluation of the client's:
Client visit report – All myNurse visits result in a comprehensive client visit report that can be provided to the client, their family and any other health care advisors deemed necessary. It provides details about the client's current condition of health and daily living challenges. The report will also include any recommended follow-up actions. Most importantly, it gives everyone valuable reassurance and peace-of-mind. Crisis management home visits – People who purchase the monthly myNurse service are entitled to receive three no-cost crisis management home visits per year providing personalized support in arranging care, returning home after a hospital stay and dealing with non-emergency health problems or medication advice. Bi-weekly phone check – People who purchase the monthly myNurse service are entitled to receive a bi-weekly phone check call, a convenient service for elderly people who live alone and have health conditions that must be monitored. Care planning and provision of home care services – Assistance can be provided to myNurse clients requiring additional health care and medical services, planning for their ongoing care, help navigating the health care system for care funding and assistance in arranging a wide range of in-home care services that can be purchased separately from Bayshore Home Health.
