CareChart: After‑hours support for oncology patients, when it matters most

A cancer diagnosis often brings questions and concerns that can’t wait until morning. When patients need support after hours, having timely guidance can make a meaningful difference. That’s where Gemma Agustin comes in. Agustin is an Oncology Nurse and Director of Clinical Management with Bayshore HealthCare’s CareChart, a program that partners with Ontario cancer centres to provide after‑hours support to oncology patients.

H2H program in Sudbury supports patients and the healthcare system

In partnership with St. Joseph’s Health Centre, Bayshore HealthCare is pleased to support the Hospital to Home program and work closely with patients on their recovery journey.

Bayshore HealthCare achieves “Accredited with Exemplary Standing” status from Accreditation Canada

Delivering care for medically complex children requires the highest clinical standards and deeply skilled teams. Bayshore is proud to share an accreditation milestone that reflects our pediatric nursing teams’ commitment to safety, quality, and family‑centred care.

Established in 2003, the Bayshore Pediatric and Specialty Nursing program in Burnaby, British Columbia delivers nursing support services through in-home and in-school respite nursing care for children and youth with medically complex and fragile health needs.
