Bayshore HealthCare partners with North York General Hospital to provide an Alternative Level of Care Transitional Rehab Program
MISSISSAUGA, ON (March 20, 2019) Partnering with North York General Hospital (NYGH) and Queens Estate Retirement Community, Bayshore HealthCare opened an Alternative Level of Care (ALC) Transitional Rehab program designed for patients whose hospital treatment plan is complete but who are unable to return to their home right away. Eligible individuals receive high-quality comprehensive care, in a supportive congregate community setting.
Funded by the Central Local Health Integrated Network (LHIN), 20 transitional care beds opened at Queens Estates Retirement Community on February 13th, 2019, and are available through to April 2019.
The Transitional Care Rehab program can help reverse the effects of de-conditioning for patients who have had extended stays in hospital, and prepares them to return to community living. The program is not a permanent destination. The goal of the unit is to provide patients with high-quality 24-hour care until they can safely return home. The typical length of stay for patients is between 1-45 days.
By receiving care in the Transitional Care Unit, eligible patients have access to many restorative therapies as well as nursing, physiotherapy, occupational therapy and personal care support to help them prepare for life at home again. This environment also provides time for patients and their families to receive additional education on how best to care for themselves after discharge.

The program fills a gap in the current health system by providing intensive rehab and restorative care to patients in the program. The key objective of the program is to ensure that patients and their families have all of the proper supports and services in place for a smooth and successful transition home. Additionally, the program aims to reduce the pressure on ALC beds, unscheduled emergency department visits and hospital readmissions within the Central LHIN by increasing community supports.

Health professionals make this transition as seamless as possible by enhancing their abilities and focusing on their strengths.”
