Nurse Practitioner (NP) Program
Bayshore’s Nurse Practitioner (NP) program is an innovative model for delivery of comprehensive health care in the comfort of your own home. The NP program is designed to improve access to care for you or your family member who may not have a Family Doctor or who does not want to risk leaving the safety of your own home to receive care. Our nurse practitioners are experts in home and community health care needs assessment, planning, interventions and evaluation.
One of the unique aspects of the NP program is the focus on wellbeing, health promotion and disease prevention. The day-to-day delivery of care by one of our nurse practitioners, in the comfort of your own home, may include:
- Diagnosis and treatment for common illnesses and injuries
- Ordering and interpreting diagnostic tests ( such as X-ray and/or blood work)
- Support in managing a chronic condition (such as diabetes, high blood pressure, asthma)
- Prescriptions for medication
- Regular check-ups including physicals and routine screening tests (such as for falls, immunizations)
- Mental health counselling and education
- Referrals to health care specialists who can help with a specific condition
- Support in navigating the health care system
Bayshore Nurse Practitioners can also support your care needs virtually, when and where you need it.
In addition to nurse practitioners, the NP program includes services from other professionals such as registered nurses, social workers, pharmacists, dieticians, occupational therapists, physiotherapists and others. The design of each team is based on your individual care plan needs.
Currently available in Mississauga-Halton, the NP program assists clients in Halton Hills, Milton, Oakville, Northwest Mississauga, Southwest Mississauga, East Mississauga , South Etobicoke and Hamilton area.

‘Nurse Practitioners (NPs) are licensed by jurisdictional nursing regulators. NPs are graduate prepared health providers who practice autonomously and independently. NPs provide direct care to patients to diagnose and manage disease/illness, prescribe medications, order/interpret laboratory/diagnostic tests, and initiate referrals to specialists.
